The health and welfare of everyone on campus is of vital importance to the faculty and administration. Please feel free to confer by phone or in-person with the School Nurse or your child’s Division Head on any matters relative to your child’s health and well-being. Medical Admissions Requirements

All students accepted to Saddle River Day School are responsible for meeting all state and school medical requirements.
These include submitting the following BEFORE the first day of classes:

Annual Physical Examination and a copy of student's immunization records
Student Emergency Form
Medical Administration Authorization Form
Allergy/asthma/seizure emergency action plans if applicable.
Parents/guardians must bring in emergency medication to be kept in the health office on the first day of school unless students are approved in writing by their medical provider and a parent/guardian to self-carry.

Immunization Requirements

New Jersey requires all students to receive certain vaccines before attending school. A summary of NJ school required immunizations can be found here.

The New Jersey Department of Health (NJDOH), Vaccine Preventable Disease Program also mandates the following immunizations:

Preschool

Children six months through 59 months of age attending any licensed child care center, or preschool facility on or after September 1, 2008, shall annually receive at least one dose of influenza vaccine between September 1 and December 31 of each year (N.J.A.C. 8:57-4.19) Kindly send your child's documentation of a flu shot to the school nurse by December 31st of the current calendar year.

6th Grade

Meningococcal and Tdap vaccines are required for all entering sixth graders who are 11 years of age or older. If a child is in sixth grade and under age 11, he/she must receive the vaccines within two weeks of their 11th birthday.

Physical Exams

Lower School

All students from PreK-3 to 4th Grade must submit an annual physical and most recent immunization records prior to the first day of school.

Middle/Upper School

All students in 5th grade to 12th Grade must submit an annual physical and most recent immunization records prior to the first day of school.
Should your student wish to participate in athletics beyond regular PE, please use the forms found on the Athletics page of our website and submit a completed copy to the School Nurse.

Athletes

Student athletes must submit the Medical Eligibility Form to the school nurse in order to be approved for a sport. The Physical exam must be completed within 365 days of the 1st day of practice for the sport they have registered for.
If a student has had a physical exam more than 90 days prior to the first day of official practice, they must also provide a health history update questionnaire completed and signed by the student's parent/guardian.

International Students

All students coming in from outside of the United States must complete their physical examination by a US based medical provider before the first day of school.
All students coming in from outside of the United States must also provide a copy of their immunization records for review. All immunization records submitted by a parent/guardian in a language other than English are required to be accompanied by a translation sufficient to determine compliance with the immunization requirements.
Students entering school in New Jersey for the first time and were born in a country with a high incidence of tuberculosis as determined by the Centers for Disease Control and Prevention must provide documentation of a tuberculosis test by either a Mantoux tuberculin skin test or an interferon gamma release assay blood test before the first day of school.

Illnesses

To prevent the spread of contagious illnesses, students who show signs of obvious illness should remain at home with appropriate care arranged by the parent.
Any student who is not feeling well should report to the Nurse’s Office. If a student is too ill to remain in school, the parent or guardian will be contacted by the nurse, and they will be dismissed from the Nurse’s Office.
Any student out of school for illness-including surgery, injury, dental, or medical for 3 or more days, must have a signed doctor's note allowing the student to return to school.

As per the NJ Department of Health, you MUST keep your child home if there is:

Fever of 100 Degrees Fahrenheit or more
- Must be fever free without the use of medication for 24 hours before returning to school.
Vomiting or diarrhea
- Must be free of these without the use of medication for 24 hours before returning to school.
An unidentified rash
- May return after cause has been identified by a medical provider, and it has been 24 hours after starting treatment if prescribed.
Redness to the eye(s), especially if there is crusting and/or drainage
- May return after cause has been identified by a medical provider and it has been 24 hours after starting treatment if prescribed and if there is no further drainage.
Persistent sore throat or cough
- May return after cause has been identified by a medical provider and it has been 24 hours after starting treatment if Strep Throat has been diagnosed.
Lice
- May return after proper treatment and there are no nits or lice present. Children must be brought to the school nurse by a parent to be checked and cleared to return to school after either being sent home for lice or if they are found during non-school hours.

Injuries

All students using crutches, splints, immobilizers, or casts must be reported to the School Nurse prior to going to their classroom.
A physician’s note MUST be submitted to the School Nurse with the diagnosis and any restrictions or limitations for Physical Education, sports, recess, class activities, writing, and testing. This note must include length of time for any restrictions/limitations.
Any student with restrictions due to injuries MUST provide a physician’s note for clearance to return to activities such as Physical Education, sports, or recess.
All injuries received on campus or in a school-sponsored activity must be reported to the School Nurse, appropriate division head, teacher, or coach. This is vitally important to expedite emergency treatment of the injury, provide follow-up care, communicate with the parent(s) and/or guardian(s), and promptly compile information for school insurance purposes.

Medication Administration

NO medication will be administered in school without the proper completed forms. This includes all prescription/over-the-counter medication administered in school. Students may not self-carry any medication except for those approved in writing by their doctor and a note must be sent to the School Nurse.

Medication for life-threatening conditions

Students with life threatening allergies may carry and self-administer epinephrine if approved in writing by their medical provider. An Allergy Action Plan must be completed by a parent/guardian and the student's medical provider and submitted to the School Nurse.

Students with asthma may carry and self-administer inhalers if approved by their medical provider. The Asthma Action Plan must be completed by the student's medical provider and submitted to the School Nurse.

Individual Health Care Plans

An Allergy and Anaphylaxis Plan is required for students who have a life-threatening allergy. It is recommended that students self-carry and learn to self-administer epinephrine as they begin to prepare for high school.
An Asthma Action Plan is required for students who have asthma and will be using an inhaler or nebulizer at school. Students at the middle school level and above are permitted to self-carry asthma inhalers if the Asthma Action Plan states that the student is capable of self-administration.
A Diabetic Medical Management Plan is required for students who have a diagnosis of diabetes.
A Seizure Action Plan is required for students who have a seizure diagnosis. This plan provides instructions for seizure management in the event of a seizure in school.
