Please complete the form below for each child who will be using after care.  After care begins Monday, September 9.

After Care Cost: $550day for the year. (1 day is $525, 2 days $1075, 3 days $1600, 4 days $2125, 5 days $2650). There is a 25% sibling discount. 
 
Drop-In Cost: Drop-Ins will only be permitted based on availability for that day. The cost is $35/child per day.
 
After Care Hours: 3:00 pm-5:30 pm. After Care will begin on Monday, September 9th. 
 
Please note, to insure a low student:staff ratio, after care will be limited to 20 Pre-K-K and 25 Grades 1-4 per day.

After Care Dismissal Information

The following people have my permission to pick my child up from school.


Please only include people picking up your child on a regular basis.  A written note of permission will be necessary for anyone not on this list (for example: play dates). Include all people authorized to pick up your child including yourself and/or your spouse.
 

Saddle River Day School has permission to dismiss my child to all the people listed above.
