Athletics Forms

Questions about the forms below can be addressed to PJ Petrow, Athletic Director - ppetrow@saddleriverday.org.

To participate in athletics at SRDS in grades 5-12, you will need to complete the following steps:

1.  Register online with Family ID via the link below.

2.  Print out the forms below and bring them to the physician conducting the physical.

3.  For upper school sports, completed forms must be submitted to Nurse Lei Ann McChesney (lmcchesney@saddleriverday.org) by August 9.  For middle school sports, forms must be submitted by August 27.
